The Ampersand Care Portal is an NHS-approved solution designed for hospital teams to monitor and stay connected with their patients. We enhance healthcare efficiency and empower clinicians to provide tailored treatment by leveraging comprehensive data collection and remote monitoring capabilities. This dynamic approach ensures that patients receive timely and personalized care precisely when they require it most.

The landing page of our Care portal, known as the "Dashboard," provides clinicians with a comprehensive overview of their daily tasks. It prioritizes the main tasks and their actionable items, ensuring easy accessibility and a streamlined workflow. If clinicians require additional information to proceed, they can conveniently access the patient records, with all the necessary clinical data.

While accessing patient information from the tasks may appear straightforward, it's important to note that many NHS systems and records operate differently, clinicians have to search individually for patients before and taking action after, sometimes using two or more different apps.

The Todo list page shows each "Care Plan" item. It displays every patient's current, upcoming, or missed item from their care plan. In the comprehensive list view (shown below), the actions are categorized by type and arranged in columns based on their urgency. To simplify navigation and streamline the process, we have incorporated appropriate filtering options for both the type of action and the urgency. These features are crucial in supporting clinicians and enhancing the daily lives of their patients, enabling them to focus on their actual tasks with greater ease and efficiency.

Two way messaging
Two way messaging

The two-way messaging feature has been a long-planned addition to the Care portal and mobile apps. My goal was to provide patients with a direct and instant connection to clinicians and clinical care, without adding pressure to hospitals but rather assisting them in utilizing their time more effectively. To achieve this, I devised and began validating solutions.

One approach I devised was collaborating with clinicians to create help topics that address the most common issues. This enables us to provide immediate guidance within the app, helping clinicians prioritize their tasks. Additionally, if the issue is serious, it offers clinicians some context before they proceed with the necessary actions.
